Fraud Reporting Center – Incident Submission

  • Thank you for using the Clark Nuber Fraud Reporting Center to report fraud at your company. Remember: you do not have to share your name or contact information with us to report a fraud, but if you do, your information will not be shared with your company.

    Before you begin, please be certain that your company has registered for this service. If you are uncertain whether your company has registered for this service, please ask someone you trust, or if you do not require anonymity, contact Pete Miller.

    If your company has not registered to use this service, we cannot process the information. Please visit the Office of the Comptroller of the Currency website for other fraud resources.

    DISCLAIMER: This Fraud Reporting Center and online reporting form are designed for reporting suspected cases of fraud and are not a substitute for employment-related grievances or appeals. You may remain anonymous if you prefer. All information received or discovered will be discussed only with persons having a legitimate reason to receive the information.

    NOTE: Upon submission of this form, the information contained in the fields Incident Category, Involved Parties, and Details of Incident will be copied in their entirety, without additions, deletions or alterations of any kind, and forwarded to all designated contacts named by your company, unless they were named by you as an Involved Party. In the case a designated contact is named by you as an Involved Party, these contents would not be forwarded to that Involved Party and instead would be forwarded to all other designated contacts. These contents will be forwarded no later than 5 business days after submission.

  • Choose the type of fraud you suspect at your company. Choose the category that best describes the fraud you suspect.
  • List the names and titles of the individuals you believe to be involved in fraud.
  • Provide as much detail about the incident as possible, including when you believe it started, when you discovered it, and how it is being accomplished.

  • The following optional fields for your contact information are there so we can follow up with you if we need more information or if you would like notification when the incident has been reported. If you would like to provide contact information, we recommend providing your personal email address and phone number to reduce the possibility that your identity will be revealed to a possible perpetrator.
